Super Geriatric - (Ages 80+) reducing the parentification of our children. Loss of independence, when to give up driving, when to move and how to get help when needed. Decreasing isolation and loneliness after loss. Growing social supports after loss.

Disease management (Any age) - Parkinson's, Diabetes, End Stage Renal Disease, stroke, etc are just some of the diseases that can impact functioning, quality of life and identity. There is no shame in getting help to manage the emotional and practical challenges of chronic and acute illness. Planning for outcomes, what to expect, where to find help and having a voice in care management. Building effective communication with care providers, doctors, nurses and social workers. Case management services are available upon request. Caregiver supports to prevent burn out.
